Small milligrams, same QT avoid list

Levitra labels tell patients with congenital QT prolongation, and anyone on Class IA (quinidine, procainamide) or Class III (amiodarone, sotalol) antiarrhythmics, to avoid vardenafil. That sentence does not say avoid 20 mg but 2.5 is fine. This desk's 2.5-5 mg lock is still inside the avoid.

Thorough QT work showed modest QTc increases at labeled and supratherapeutic doses. Clinical relevance depends on other risk - low potassium, other QT drugs, structural heart disease. The practical desk move is simpler: do not add a known QT-active PDE5 to a known QT-active antiarrhythmic for an erection.

CYP3A4 inhibitors raise vardenafil levels and, with ritonavir, stretch half-life toward a day. More exposure for longer is the opposite of a cautious 2.5 mg story if amiodarone is already lengthening the interval.

Sildenafil and tadalafil have their own cardiac cautions but are not interchangeable safer-QT DIY swaps without a clinician. This page will not bless a silent switch to Cialis 20 mg on Friday to dodge a Saturday Levitra QT talk. That is a visit.

Nitrates remain forbidden. A QT conversation does not open a nitroglycerin window.

Wanting a different PDE5 is a clinic sentence

Patients ask to just use Viagra 25-100 instead to escape the Levitra QT paragraph. Sildenafil has cardiac cautions of its own and still forbids nitrates. A clinician can choose it. A reader cannot declare QT solved by a logo change.

Tadalafil 20 mg adds a 48-hour nitrate hold. It is not a stealth QT holiday. Long tail plus any residual antiarrhythmic is a longer overlap if someone tries both in one weekend.

Stopping amiodarone to open vardenafil is a cardiology decision measured in weeks and tissue stores, not a Thursday skip.

Export Levitra-style 20 mg blisters do not carry a cleaner QT profile than 5 mg film-coated originator stock. Strength and brand do not rewrite Class III.

Inhibitors lengthen the hours you sit on the interval

Ritonavir's 2.5 mg every 72 hours exists because exposure explodes. If someone ignored the QT avoid and also takes a booster, they have built a long, high vardenafil curve on purpose.

Ketoconazole 400 mg daily parks vardenafil at 2.5 mg per 24 hours. Azoles can have their own QT stories. Two QT-adjacent drugs plus a PDE5 is a specialist problem, not a 2.5 mg comfort.

Erythromycin raises vardenafil and is itself QT-discussed in infectious-disease practice. A sinus course plus 5 mg Levitra is not only a CYP cap. Ask the prescriber about both axes.

Grapefruit is a small CYP nudge. It is not an excuse to skip the IA/III list. It is another reason not to treat 5 mg as harmless.

How cardiology and the ED prescriber must share one list

Two clinics that each know half the list will assemble vardenafil plus amiodarone by accident. The ED letter should name the PDE5. The cardiology letter should name the Class IA or III agent and the last QTc. One shared problem list beats two polite silos.

Discharge after new sotalol should include an explicit PDE5 hold if vardenafil was a home drug. 'Resume home meds' is how 5 mg returns on Friday.

Infectious-disease starts of clarithromycin or a booster should ping the same list. CYP stretch plus QT avoid is one note, not two afterthoughts.

Patients who shop a new 'fast' brand after a QT lecture are still on vardenafil if the INN did not change. Export lightning art is not a different molecule.

If nobody will own the combination, the combination does not exist. Default is avoid. Traces do not referee specialists into a yes.

A gastroenteritis week is a QT week

Vomiting, loop-diuretic bursts, and poor intake drop potassium. Adding 2.5 or 5 mg vardenafil that week is a chemistry stack even if the antiarrhythmic dose did not change.

Bananas are not a same-evening assay. Recheck electrolytes with clinic. Skip the PDE5 until the number is boring.

Magnesium belongs beside potassium on the same lab slip if the rhythm clinic already watches both.

Alcohol-heavy nights that end in vomiting are a double story: hypotension pairing plus electrolyte pairing. Do not treat the hangover with a 'small' 2.5 mg to keep a wedding plan.

Bring the last potassium date to the parent orbit visit the same way you bring the amiodarone dose.

If the rhythm clinic already scheduled a QTc next month, do not run a first-ever 5 mg experiment the week before that ECG. The trace can wait. The interval measurement should see your baseline, not your leftover vardenafil plus a new faint story. Bring the blister to that ECG visit even if you did not swallow - so nobody assumes the home list is only amiodarone.

When the interval conversation becomes 112

Syncope, palpitations with near-collapse, or seizure-like loss of consciousness after a first vardenafil dose on a QT-active list is emergency care. Bring the blister.

Chest pain still follows the nitrate rule. Do not let a QT focus hide a forbidden spray.

Priapism and sudden vision or hearing loss remain class stops. They are not proof the QT pairing was fine.

A fast peak is the worst time to add a QT partner

The onset page celebrates an early Tmax. Electrically, an early peak is when you would rather not also be loading sotalol's nadir. Do not schedule first-ever vardenafil on the evening a Class III dose also lands if anyone was foolish enough to combine them.

Food delay does not make the QT pairing safer. It only moves the peak. A late peak plus a bedtime antiarrhythmic can align in sleep.

Redosing because the first 40 minutes felt empty - already forbidden on the onset page - is worse if QT drugs are present. Two peaks, one night, one interval.

Alcohol and heat still drop blood pressure. Syncope from hypotension can be misread as arrhythmia. Emergency care, not a forum differential.

What to bring if anyone still wants vardenafil on a messy list

Latest ECG date and QTc if known, potassium and magnesium if recently drawn, exact antiarrhythmic names and doses, CYP3A4 partners, and the 2.5 versus 5 mg question you are actually asking.

Family history of sudden death or known long QT belongs on the same page.

A normal ECG in 2019 is not a 2026 clearance for a new PDE5 plus new sotalol.

This desk will not read your ECG from an email photo. We will say the pairing names.

  • Spell IA/III names; do not write rhythm tablets.
  • List boosters and azoles beside the QT drugs.
  • Do not stop amiodarone to make room for 5 mg.

Name the tablets, not heart-rhythm pills

Clinic notes that say antiarrhythmic without the class waste the QT row. Quinidine and procainamide are IA. Amiodarone and sotalol are III. Those four names should be on the vardenafil intake, spelled.

Other QT-prolonging medicines - some macrolides, some antipsychotics, some methadone regimens - are consider territory in broader QT hygiene even when they are not the named IA/III forbid. This desk will not invent a complete QT encyclopedia. It will say: bring the full list, not the ED strip only.

Hypokalemia after a loop diuretic or vomiting makes any QT-active stack worse. A 5 mg Levitra after a gastroenteritis weekend is a chemistry problem as much as a drug-drug problem.

Congenital long QT is a family-history and ECG diagnosis, not a vibe. If a relative has it and the patient has never been checked, that is an ECG conversation before any vardenafil, including 2.5 mg.

Write the antiarrhythmic class, the last ECG, CYP3A4 partners, and the 2.5-5 mg question on one slip. File it against the vardenafil orbit.

Peak timing without QT drugs stays on the onset orbit. Prescribers, not traces, decide whether any PDE5 is open.

The label's avoid is not dose-tiered for 2.5 versus 20. Stable amiodarone is still Class III. Do not swallow vardenafil to test a loophole. Ask cardiology and the ED prescriber for a different molecule or a documented rare override. This trace will not grant the exception.

Not tonight as a self-swap. Sildenafil still forbids nitrates and still needs a partner review. A clinician can move you to the 25-100 ladder. You cannot declare QT solved by changing logos. Read the sildenafil orbit only as study, then book the visit.

The 2.5 mg / 72 h row is a CYP cap, not a QT waiver. Booster plus Class III plus vardenafil is a triple you do not assemble from traces. Both specialists need the same med list. Do not start 2.5 mg from a forum interval.

Sotalol's effect is not a twelve-hour guest that leaves before bedtime. Class III plus vardenafil is an avoid, not a scheduling puzzle. Do not split the clock to invent safety. Take the pairing to clinic. If they want a PDE5 at all, they choose it; you do not time-slice sotalol.

Tadalafil is not a vardenafil QT loophole you award yourself. It brings a 48-hour nitrate hold and its own cardiac cautions. A prescriber may choose it. You may not stack it with leftover vardenafil or with the same IA/III drugs as a hobby. See the tadalafil orbit only after they pick an INN.

Intermittent Class IA is still Class IA. Rarely is not a clearance. Show the bottle to clinic and get a current rhythm plan before any vardenafil. Off-day folklore is how people stack on a Friday they forgot they dosed Thursday.
